Primary Purpose of Organizational Unit: The Community Alternatives Program is a Medicaid funded service which allows individuals to receive services in their home to avoid long-term care. The Community Alternatives Program for Disabled Adults (CAP/DA) is a special Medicaid program under the Community Care Section of the Division of Medical Assistance. Maple Leaf Adult Respite is an adult daycare/day health program serving adults with disabilities in an effort to meet their social and medical needs. This program provides respite to caregivers and allows individuals to be in a safe environment during the day.


Primary Purpose of the Position: Primary purpose is to provide nursing services and care coordination to individual clients within the Maple Leaf Adult Respite facility, and secondarily, to provide nursing assessments for the Haywood County Health and Human Services Agency Community Alternatives Programs (CAP/DA, CAP/C).


Through onsite assessment, consultation, and health education, the Maple Leaf Respite /Community Alternatives Program (CAP) Nurse will help to create and support environments that best promote the health and well-being of the population served. Secondly, provide nursing assessments for the Community Alternatives Programs (CAP) Program per program requirements. Provide general health clinic services and education to clients served. This nurse will:


  • Have knowledge and understanding of the physical and emotional aspects of aging, the resultant diseases and infirmities and related medications and rehabilitative measures

  • Assess the health and safety needs and practices of clients

  • Conduct physical assessments of clients

  • Assess and educate clients about basic care, skills, and abilities

  • Connect families with community resources

  • Establish and review health policies and procedures

  • Coordinate with community based health care providers

Knowledge, Skills and Abilities: Basic knowledge of, and skill in, the application of nursing theory, principles, and techniques employed in the field of public health and related programs; understanding of best practices, safety standards, and therapeutic outcomes related to the specific area(s) of practice; working knowledge of current social and economic problems relating to public health, including health disparities; working knowledge of available resources and organizations; ability to deal tactfully with others and to exercise good judgment in appraising situations and making decisions; ability to work in partnership with patients and with other service providers to elicit needed information and to maintain effective working relationships; ability to accurately record services rendered and to interpret and explain records, reports and medical instructions; some ability to oversee the work of others; adequate computer skills to allow communication, patient record documentation, accessing information.


Minimum Training and Experience Requirements: Graduation from a four-year college or university with a B.S. in Nursing which includes a Public Health Nursing rotation, or Master’s in Public Health and graduation from a school of professional nursing; or graduation from a school of professional nursing and one year of professional nursing experience; or an equivalent combination of training and experience.


Training/Experience: Graduation from an accredited school of professional nursing.


Necessary Special Qualifications - A current license to practice as a Registered Nurse in North Carolina by the North Carolina Board of Nursing.
